Abstract

Patient safety remains at the forefront of healthcare concerns across the world. Complex healthcare environments demand constant vigilance to prevent adverse outcomes. Medication safety constitutes a critical aspect of patient care. Since nurses handle the administration of most prescribed medications, the responsibility largely falls to them. Moreover, irregularities in nursing practices account for nearly 44% as a leading contributor to medication errors (Elena Herrera, 2015). Pharmacists complement these efforts by providing expertise and oversight. The two professions combine to play a pivotal role in patient safety. In the past, the professions often worked independently. More recently, collaboration has become a priority among healthcare institutions. Notebook pages iterate findings from extensive bibliographic consultations elucidating the importance, benefits, and impediments of cooperative efforts, along with practical strategies to enhance teamwork. Collaborative partnerships generate positive outcomes for patients while ensuring safety. Integration remains vital to the comprehensive delivery of patient-centered care.
